It wasn't an "attack" the fish just leapt and accidentally speared her. The fish is way too fast and agile for her to even attempt to approach and threaten it. It's just a very rare and sad accident.
I had a dolphin flying on a wave I was on land on my head and flip me off my board and over the falls. The ocean is dangerous, gotta stay on your game.
